✅ Check the simple stuff first
- Make sure the patient is safe — swap in a backup unit if you need to.
- Reproduce the complaint yourself and write down exactly what you see (screen, alarm, code).
- Check power, battery, cables, accessories, and settings before ordering parts.
- Then open the service manual above and follow its troubleshooting section.
🛠️ Common faults for Medfusion 4000 (8)
Occlusion - Check Infusion Line source ↗
- Look for closed stopcocks/clamps, kinked tubing, IV site patency
- If frequent with no real occlusion: tubing may be too small for rate/viscosity; consider FlowSentry or another occlusion setting
- If it can't be cleared, send for biomed repair
Syringe flange / plunger sensor alarms source ↗
- Check barrel clamp, flange holder and plunger holders are engaged and move freely
- If holder misbehaves or alarm persists, remove from service
Motor Not Running / Motor Rate Error / Force Sensor test source ↗
- Power off and on
- If it persists, remove from service for repair
'System Advisory - Maintenance is Recommended' source ↗
- PM reminder set in Biomed menu; send for PM and reset
Syringe loading sensor alarms (Barrel Clamp / Flange / Plunger) source ↗
- Engage the barrel clamp, flange holder and plunger holders; check they move freely
- Persistent or sticking holders: remove from service
Frequent occlusion alarms with no real blockage source ↗
- Set tubing too small for the rate or viscosity; enable FlowSentry or change the occlusion setting in Configuration
- Use the smallest syringe for the volume (low rates on large syringes delay occlusion detection)
Motor Not Running / Motor Rate Error / Force Sensor Bgnd Test source ↗
- Power cycle; if it recurs, biomed repair
Biomed Passcode screen appears after a system fault source ↗
- Remove from service immediately for inspection or repair
🔢 Medfusion 4000 error codes (15)
| Code | Meaning | Next step |
|---|---|---|
| Check Syringe Barrel Clamp | Clamp sensor can't confirm position, or the clamp was resting on the handle at power-up | Verify the clamp is on the barrel; Confirm source ↗ |
| Check Syringe Flange Sensor | Syringe flange not loaded correctly | Re-seat the clamp, flange holder and plunger holders; if the holder sticks, remove from service source ↗ |
| Check Syringe Plunger Sensor | Plunger not loaded correctly | Re-seat the plunger holders; they must move freely; otherwise service source ↗ |
| Force Sensor Bgnd Test | Force sensor background test failed | Silence and restart; if it recurs, service source ↗ |
| Invalid Syringe Size | Size not recognized, or the clamp was lifted during delivery | Don't lift the clamp; check the supported sizes; service if it persists source ↗ |
| Motor Not Running / Motor Rate Error | Sensor, mechanical or electrical fault | Power cycle; if it persists, service source ↗ |
| Occlusion - Check Infusion Line | Line blocked | Clear stopcocks, clamps and kinks; restart; if frequent with no real occlusion, the set is too small for the rate or viscosity: consider FlowSentry or a different occlusion setting source ↗ |
| Pressure Increasing - Check Infusion Line | FlowSentry detects rising pressure | Clear the line before it reaches the Occlusion alarm source ↗ |
| Restricted Flow - Bolus/Loading Cancelled / Rate Reduced | Restricted flow during bolus or loading dose | Check the line for occlusion source ↗ |
| Calculated Rate Out of Range / Invalid Infusion Parameter Combination / Invalid Entry | Invalid program or Drug Program | Verify the entries; have the PharmGuard Toolbox 2 admin fix the library source ↗ |
| Pump is Locked | Keypad locked | Press Unlock source ↗ |
| System Advisory: Low Battery / Low Battery Change | Battery low; alarms again each 1% drop | Plug in source ↗ |
| System Advisory: internal battery not working | Battery failed or depleted | Use on AC; service soon source ↗ |
| System Advisory: base hardware/software failure, invalid configuration, network settings corrupt | Base or configuration fault | Remove from service as soon as possible source ↗ |
| System Advisory: improper power-down / Time and Date lost / PM due | Housekeeping | Schedule service source ↗ |
🧰 Common checks for Infusion Pumps (5)
Occlusion source ↗
- Check for closed clamps/stopcocks, kinks, IV site patency
- Check pump height vs patient (LVP)
Air-in-line source ↗
- Look for small bubbles; disconnect and re-prime
Door/cassette/syringe loading source ↗
- Close door with set seated; check syringe clamp, flange and plunger holders
Battery/AC source ↗
- Run on AC; replace/maintain battery; battery service alarms → repair
Device/system error source ↗
- Power cycle once; if it repeats remove from service
